Work with the employer’s benefits coordinator to get **CMS L564** completed to upload in step 2 for proof of employer group health insurance.

Apply for Medicare A+B or Part B only if you already have A.

Upload CMS L564 when you apply thru Social Security to prove you have a special election period being over 65 years & 4 months old coming off creditable coverage through an employer with over 20 employees as shown in the CM L564 form completed by the employer in step 1.

Once you have your Medicare number with A+B effective dates, schedule an appointment with me before the month you need coverage to begin.

**Medicare A+B** **are required** to secure a Medigap or Medicare Advantage plan.
