Complete these 3 steps before the month you need coverage to begin.
Obtain CMS L564
Work with the employer’s benefits coordinator to complete a CMS L564 from Social Security for each individual applying for Medicare Part B. If multiple employers provided coverage from the time you turned 65 to present, obtain a form from each employer to prove there was no gap in coverage.
When you apply thru Social Security, attach proof of employer group health insurance from step 1
(CMS L564) to prove the right to apply and avoid penalties.
Schedule an appointment or reach out to enroll for additional coverage before the month you need coverage to begin.
Medicare A+B are required to secure a Medigap or Medicare Advantage plan.
